TA Evaluation Empty TA Evaluation

Hey all.

My fellow teachers at Pingyang school are interested in setting up an evaluation form for the TA's to help improve relations.

I have thought of a few areas for the evaluation to be on but was wondering if anyone can offer further help?

What are the most important points for the TA in the classroom?
